AVI Tags & Decals
A valid AVI tag and windshield decal are required to be installed on each commercial vehicle operating in the Ground Transportation Program.
There is a non-refundable fee of $25.00 for each new or replacement tag installed. Only permitted operators should submit requests through this form. Unpermitted operators must use Infrequent Operator Permits for their vehicles.
To have a new or replacement tag installed, follow these steps:
- Pay Balances
- Invoices
- Can be paid online through the e-Bill portal.
- Call Kim Nguyen in Airport Finance 408-392-3656 or call City Finance 408-535-7055.
- Do not contact the GT Office to make a payment. We cannot process payments.

The company must submit an updated insurance form showing coverage of all active vehicles, including the vehicle to be tagged. This is usually the “ACCORD” or "Certificate of Insurance (COI)" and is available from your insurance provider. The ACCORD/COI form must also reflect the "City of San Jose as an additional insured under auto liability". Norman Y. Mineta San Jose International Airport should be listed as the Certificate Holder. Complete the AVI/Decal Installation Request Form, including your Insurance Accord as an attachment. - Scheduling an Appointment
Once your request is approved, a link will be emailed to you to schedule the installation(s).
