To close your permit, please:
- Open the Voluntary Permit Closure form. The form is in Portable Document Format (PDF). You will need Adobe Acrobat Reader or other compatible application to view it.
- Type in your changes and print or save the form.
- Email your completed form (as an attachment) to airportgt@sjc.org or fax it to the number listed on the form.
