A valid AVI tag is required to be installed on each commercial vehicle operating in the Ground Transportation Program.
There is a non-refundable fee of $25.00 for each new or replacement tag installed.
To have a new or replacement tag installed, follow these steps:
- Prepare documents
- Insurance
The company must submit an updated insurance form showing coverage of all active vehicles, including the vehicle to be tagged. This is usually the “ACORD” or "Certificate of Insurance (COI)" and is available from your insurance provider. The ACORD/COI form must also reflect the "City of San Jose as an additional insured under auto liability". Norman Y. Mineta San Jose International Airport should be listed as the Certificate Holder. Please download the Vehicle AVI Tag Installation Sheet to your desktop and complete all fields. Do not print and sign the form as the signature will be collected the day of the appointment. Save the completed form to your desktop as the company will need to submit the documents in the next step. Up to four vehicle tags can be installed per appointment.

Authorized Vehicle List
Please download and complete the Authorized Vehicle List Form and list ALL vehicles operating under your approved Airport Ground Transportation Permit with SJC. All fields are required and failure to complete the vehicle list in its entirety may result in the delay of the approval of your AVI tag appointment request.

Once your insurance is approved and your account is clear of any monies owed to the City, a link will be emailed to the address used in the step above.
